Please fill out all forms that are applicable to your family and return them to the Front Office before the start of each school year.
- Student Information and Authorization Form
For your convenience, forms populated with your information were sent with tuition invoices during the first week in June. Use the above link to download a blank copy of the form. It must be completed, signed on both sides, and returned prior to the start of school.
- Alternate Pick-up Authorization Form
Authorizes others (carpools, nannies, friends) to pick up a child routinely. If we do not have this form on file for your child, we will need to make contact with you in each instance someone besides a legal guardian comes to pick up your child. - Immunization Forms
For new students only. Families may also ask their previous school or doctor's office to send immunization information directly to FAIS. Contact School Nurse Paula Bergsma with any questions.
- Step-parent / Additional Family Form
For families with more than two parents or guardians. - Gilkey Student Network Acceptable Use Policy
All Middle School students and a parent or guardian must read and sign this document each year before students will be allowed to access the FAIS computer network.
