Student Information and Authorization Forms Due June 21

Please turn in these forms, as well as your 2012-13 Alternate Pick-up Authorization (the 2011-12 form will no longer be valid in September 2012), prior to the last day of school. Returning your forms on time is critical for updating the School’s database for the 2012-13 school year. There are four options for returning these forms to school:
- Deliver forms directly to the Front Office
- Scan and email signed copies to school
- Return via snail mail
- Drop them off at the curbside courtesy collection Monday through Wednesday, June 18 – 20 (mornings only)
